I printed out one from our previous yahoogroup name, but here is a link to where it is for you to print out and get Dr. Roe to sign for you.  My family prefers to go to Golden Corrall when we all go out to eat and I use it there.  Haven't used it at other places because I will just get a container to take whatever is left over home and have it for several more meals or give it to my husband.
