At UMHB we have a fairly standard candidacy period for our incoming
students, followed by application to the undergraduate professional
program which lasts 3 years. Application to the program is made in
April of each year.

Are there institutions and programs using other models? If so, what
are they?

I have occasionally heard about rolling admissions processes and
limited observation periods, but I have not talked with any one in
those types of programs so I'm not sure whether they exist.
