This is a revision of a previous notice:

There is a new prioritization form which must be completed for each individual currently on the CAP waitlist.  This tool will provide a standardized and more equitable system for assigning priority for individuals waiting to receive funding.

If you are waiting for enrollment onto the Medicaid-CAP waiver, call the OPC office.

They are not calling families to schedule appointments.  You must make the call to them.
