my daughter jami and her son- with their small dog- are looking for a
lease in the ne burbs of atlanta - gwinnett area. if you have or know
of someone who has a place they are trying to sell and want to have a
good tenant til the housing market changes or has a duplex- let us
know. she needs two bedrooms- and around $650 if possible- willing to
sign an 18 month lease.
Keep in mind most of these rates will only be increased for this year
considering that the rates have been climbing more each year. Why is
there such disparity between the lowest facility- North Fulton at
22.8% and Piedmont with the highest percent 39.6? Some will say they
do more high risk pregnancies...but many will disagree and say it is
the mindset of the staff and providers.
I think it is interesting that someone on tour at one of these
hospitals said that the tour guide said they did not know the
percentage - it was not something that they kept. This is ridiculous
as it is a statistic that they have to keep and publish! It is public
information!

Care provided at the holding area is mainly self-help with minimal
intervention from these volunteers. If there are patients who are sick
enough to require actual hands on care, a treatment form must be
started and filed. Nurses/EMT's on this team must bring a copy of
their license with them. We must be able to verify a current license.
Close-toed shoes must be worn - no sandals, Tevas, Birkenstocks, etc.,
as this is a health and safety issue. Medical Volunteers will be
staying to do any patient care that may be needed in the Holding
Medical tent once participants go to the Closing Ceremony, thus they
may not be able to go to the Closing Ceremony.

FDA Issues Health Information Advisory on
Infant Formula from China
The Food and Drug Administration (FDA) has issued a Health Information
Advisory urging U.S. consumers to avoid all infant formula from China,
after several brands sold in that country may have been contaminated
with a substance known as melamine, a chemical used in plastics.
Although sales of infant formula from China are illegal in the U.S.,
FDA officials are concerned that some formula from China may be on
sale at ethnic grocery stores here. The FDA stresses there is no risk
of contamination to the U.S. domestic supply of infant formula.
Melamine in known to cause kidney stones and other serious kidney
conditions. The FDA is encouraging physicians to report diagnoses of
kidney stones in infants of Chinese ethnicity to FDA MedWatch
(1-800-FDA-1088), Centers for Disease Control and Prevention
(1-770-488-7100), or local/state health departments.

No Need to Sweep
Whether or not women underwent membrane sweeping, overall rates of
induction, postmaturity, and prelabor membrane rupture were similar.
Membrane sweeping to move the membranes off the lower pole of the uterus,
routine in obstetric care, is performed to decrease the likelihood of
labor
induction or postmature delivery (JW Apr 11 2006). However, concerns about
premature rupture of membranes and infection have led to debate about the
merits of this practice. Investigators randomized 300 women to undergo
membrane sweeping at 38 weeks' gestation (with subsequent weekly sweeping)
or no membrane sweeping. Participants had singleton pregnancies with
cephalic presentation and anticipated vaginal delivery; women were
excluded
if they had indications for labor induction or cesarean delivery or had
contraindications for membrane sweeping. Participants and researchers were
blinded to group allocation with the understanding that this would not
constitute full blinding, as patients probably would become aware of their
assigned interventions.
Intention-to- treat analysis showed no significant between-group
difference
in occurrence of prelabor rupture of membranes (7% of women without
sweeping
vs. 12% of women with sweeping; P=0.19). Among women with cervical
dilation
1 cm, however, 9% in the sweep group had prelabor membrane rupture
versus no
patients in the no-sweep group (P<0.05). Other obstetric outcomes,
including
cesarean delivery, spontaneous labor, induction, or postmaturity, occurred
at similar rates in the two groups.
Comment: These data indicate neither great value nor great risk from
membrane sweeping starting at 38 weeks. However, subgroup analysis showed
that women with cervical dilation 1 cm during late pregnancy could be at
risk for prelabor membrane rupture in association with sweeping,
suggesting
that such women might be more sensitive to the procedure. This study
excluded women with multiple gestations or those with various obstetric
indications. Despite these limitations, the data suggest that there is no
need to sweep.

We would like to split the measles, mumps and rubella shot into the
three separate diseases and finally found someone who will do it. Ira
Katz, the pharmacist at Little Five Points Pharmacy, will administer
the shots separately on the condition that I bring him 10 patients who
will commit to paying for it. This will be an option for you if your
child is at least 12 months (or will be in the next couple of months)
and he/she has not had the MMR shot.
First, some important info.. The measles have broken out in 8 states
recently, so this is the vaccine that needs to come first. My
pediatrician (Dr. Carol Price of Clairmont Pediatrics) let me know
that the other two - mumps and rubella - can wait a long time. Merck
has the measles and rubella vaccines on backorder until mid-July - not
a long wait at all. The mumps will not be available until 2009, but
since administration of this vaccine is not urgent, that's no problem.
Ira Katz says he needs a prescription from your child's pediatrician
for the vaccine. He needs 10 patients to commit because the vaccines
come in 10-packs. The cost to you will be about $35 per shot. I
have a an Rx from my child's doctor already. I'm not sure the best
way to organize this, but let's start with this: If you are
interested, please give me your name, phone number, and age of your
child that will be receiving the shot. Then call your child's doc,
ask for an Rx for measles, and pick it up from his/her office. I
will let you know when we are close to having 10 committed. Please
respond ASAP.

PS - If you are wondering why someone would want these shots split up,
please refer to The Vaccine Book, by Dr. Robert Sears. A few people
told me that 909shot.com is another great resource, though I haven't
thoroughly investigated that site myself. I'm not going to try to
convince you either way. You're free to do the research and make the
best decision for your family.
Garry Siegel just called - he only had a minute- but he wanted to
clarify it was the practice's decision to give Margaret the week off.
Not his but the groups. Just wanted to clarify my earlier statement.
Seems he was upset that I indicated it was just his decision. He has a
copy of my "public" statement regarding her move.
Just for the record- they wanted Margaret to move to North Forsyth
with them- but her unwillingness to make that additional drive, her
desire to continue to do waterbirths and her relationship with the
staff at North Fulton caused her to want to stay.
I told him that women want options- the option of having true
midwifery practiced, a hospital staff that welcomes natural, non
interventive births as well as water births are something my clients
desire.
I have no problems with Northpointe doctors- I have been attending
births with them for over 12 years... I do have a problem with
midwives in the practice that tell my clients they don't need a doula
since they will be with them the whole time they are in labor, that
they can not get in the tub until they are 7 cm although no research
shows this to be a factor in labor, and that if their doula oversteps
their bounds she will be escorted out of the hospital. They are not
team players and therefore I don't refer to them when they stab me in
the back.
But Garry and Sean have been great doctors for my clients in the past-
yes there are issues with control we all don't like- but as far as
surgeons they are great- and I have seen Dr Lambert to be very hands
off with clients who desired that. So, although I post rarely- I
wanted to set the record straight that it was a group decision to have
Margaret not be in the office this week.
It is official! Margaret will be joining Dr. Tressa Scineaux with
Alpharetta Womens Specialists located at 1360 Upper Hembree Rd,
Roswell, GA 30076 (770) 817-1970 as of next week. Their website is

Garry Siegel decided that since she is making the move, he does not
want her in the office this week. So she has the week off and then
will join the new practice. Sherry Nored is the other midwife in the
practice. And Kaye Johnson with Isis will share the call for her
private patients. Dr. Scineaux and Dr. Fredericks share the patients
at Clinica de Mama and Kaye is with Dr. Fredericks. Margaret is at
Scineaux's practice seeing her private patients two days a week. And
she will be on call for two weeks straight when she begins. They want
Sherry to have a chance to meet her patients as well. Kaye will also
be sharing call for her private patients with Margaret and Sherry. So,
you will not get to meet Kaye ahead of time if you are Margaret's
patient but I will say Kaye practices true midwifery- so she is
incredible. Some of the most wonderful births I have ever attended
have been with Kaye. So you are in great hands either way. Margaret
says everyone will love Sherry as well.
So, if you have been a part of Northpointe's practice and you want to
be with Margaret... you need to know she is not there any longer- you
need to call and let them know you will come by to get your records
and you can call the new practice to schedule your next appointment
with Margaret or Sherry. Let the office know you are Margaret's
patient. Whoo!
And furthermore for you VBACs- Margaret said Dr. Scineaux is very
hands off with VBACs and is really cool to work with letting Margaret
do her thing... so no one breathing down her neck. Remember though the
water option is not a doctor rule it is a hospital rule- so not sure
how that will change but I feel confident it won't until Northpointe
docs are no longer at North Fulton practicing- which is not until
September 1st.

I have been selling fuzzibunz diapers for years but recently some
manufacturing changes have caused me to feel the need to discontinue

HEADLINE: Old gray haired, red faced broad with small dog found on
side of track passed out!
Yet I bet some of you might think this was true of me today- my first
official day of training! But although I chose the heat of the day 2pm
to walk on a day that was in the 70's and took my dachshund with me…I
officially clocked three miles today in one hour! My goal was 5 miles
today but I feel I was being overly zealous. That headline would have
been true if I had tried that!
Now some of you may scoff at that, only three miles! … but this old
broad has been fairly sedentary over the last few years and had breast
cancer surgery only four and a half months ago! So, cut me some slack!
I went to my surgeon's office yesterday and he said I could have the
summer off from seeing him. But, unfortunately when I went to St Louis
to my son's wedding, I tugged around my heavy suitcase too much and
seemed to have torn the subcutaneous fascia in my abdomen
(Subcutaneous fascia is a continuous layer of connective tissue over
the entire body between the skin and specialized structures such as
the muscles. It comprises an outer normally fatty layer and inner thin
elastic layer. Between the two layers lie blood vessels, nerves and
the lymphatic system.). So a bulge on the left side of my open area of
the incision seems to be with me till I have it fixed surgically. So,
it means when I have the scar revision surgery in November (I
postponed it till after the 3 day walk), I will also have to have some
internal abdominal surgery as well. But, guess what- I don't have cancer!
Ever time I want to complain about something, I remind myself of that!
So, although at the speed I walked today it would take me with the
breaks probably 8 hours of walking each day of the three day, I still
feel pumped and ready to keep working hard to be ready for the October
walk and ready to ask for money in the fight for a cure for breast cancer.
Many of you already gave me a donation and for that I am immensely
grateful! I have more than reached my goal. For those who have been
thinking about it, I ask that you take a moment and consider just the
women and families you personally know who have been affected by
breast cancer. If you just donated $1 for every woman, every husband
affected, every child affected by their mom's disease, every
grandchild affected… you get my point. We need to find a cure!
So the reason for this update- is I have a team who still needs help
although I have more than reached my goal. Some of them are poor
college students- who although can walk circles around me- may not
have the friends with expendable income that I have! And some are just
beginning to raise their money. My goal was to get four corporate
sponsors and to have my money raised by April 1st. I reached my goal.

Abstract
Background: The Valsalva pushing technique is used routinely in the
second stage of labor in many countries, and it is accepted as
standard obstetric management in Turkey. The purpose of this study was
to determine the effects of pushing techniques on mother and fetus in
birth in this setting.
Methods: This randomized study was conducted between July 2003 and
June 2004 in Bakirkoy Maternity and Children's Teaching Hospital in
Istanbul, Turkey. One hundred low-risk primiparas between 38 and 42
weeks' gestation, who expected a spontaneous vaginal delivery, were
randomized to either a spontaneous pushing group or a Valsalva-type
pushing group. Spontaneous pushing women were informed during the
first stage of labor about spontaneous pushing technique (open glottis
pushing while breathing out) and were supported in pushing
spontaneously in the second stage of labor. Similarly, Valsalva
pushing women were informed during the first stage of labor about the
Valsalva pushing technique (closed glottis pushing while holding their
breath) and were supported in using Valsalva pushing in the second
stage of labor. Perineal tears, postpartum hemorrhage, and hemoglobin
levels were evaluated in mothers; and umbilical artery pH, Po2 (mmHg),
and Pco2 (mmHg) levels and Apgar scores at 1 and 5 minutes were
evaluated in newborns in both groups.
Results: No significant differences were found between the two groups
in their demographics, incidence of nonreassuring fetal surveillance
patterns, or use of oxytocin. The second stage of labor and duration
of the expulsion phase were significantly longer with Valsalva-type
pushing. Differences in the incidence of episiotomy, perineal tears,
or postpartum hemorrhage were not significant between the groups. The
baby fared better with spontaneous pushing, with higher 1- and
5-minute Apgar scores, and higher umbilical cord pH and Po2 levels.
After the birth, women expressed greater satisfaction with spontaneous
pushing.
Conclusions: Educating women about the spontaneous pushing technique
in the first stage of labor and providing support for spontaneous
pushing in the second stage result in a shorter second stage without
interventions and in improved newborn outcomes. Women also stated that
they pushed more effectively with the spontaneous pushing technique.

November 15th I went in to Northside expecting to have a prophylactic
mastectomy with DIEP reconstruction due to my BRCA +1 status- meaning
I carry the ovarian and breast cancer gene that causes my chance of
cancer in those areas to be 87%. Since I had a hysterectomy 18 years
ago- the risk was only for breast cancer.
The surgery was an eight hour procedure. I did great through the
surgery. But due to some scarring from 18 years ago, I was only able
to have the DIEP procedure done on the left breast and for the right
breast a bit of abdominal muscle was used to help the vascular
integrity of the right breast.
I escaped the hospital on Sunday and traveled to Julie's in Athens for
my recovery. I was able to get out two drains before discharge. And
returned on Tuesday to Atlanta to get the remaining four drains out.
They were disgusting!
If I ever doubted that my surgery was a mistake- which in the midst of
pain I frequently did- the pathology report on Tuesday sealed any
regret. Although the lump I had that proved to be benign was in my
left breast. My right breast proved to have cancer cells. The best
possible cancer you can have in your breast is ductal carcinoma in
situ- DICS. So it was stage 0- non invasive, slowest growing and non
life threatening. I meet with an oncologist in a few weeks after he
has looked at the pathology report- but the thought from my surgeon is
that no further treatment will be neccessary.
I stayed at Julie's til the next Sunday and returned to my home in
Lawrenceville. Pam Hannah, my best friend lined up visitors with lunch
for a few weeks. I have been recuperating and managing to keep up with
the company emails and getting my TIVO fix on the shows while I was away!
My recovery is four to six weeks. My new "girls" look great but I am
sore and tired a lot during this healing time. This proves to me that
you need to listen to your intuitive voice- and move forward in a
proactive way. I have no regrets on my decision in any way. I look
