Hi there, I'm launching a practice and I cannot find a copy of the
form that you can give patients to submit to their insurance company
for reimbursement. The form I'm thinking of is a single page with
codes on it for acupuncture first treatment, follow up, with e-stim,
no e-stim. It also had codes for common diagnosis such as neck pain,
low back, etc...

If anyone has this form on file, would you share it with me?
