Old Dominion University is working on a project with the Virginia Department of
Health in which we are examining, among other things, the commitment, readiness,
and interest to prevent youth violence (including teen dating violence and
sexual assault) in the Commonwealth of Virginia. This is part of a violence
prevention project funded by the CDC. We are asking a series of questions
about violence prevention in the Commonwealth of Virginia in order to create a
foundation from which better understanding about the issue will evolve. I am
hoping that you may be able to devote some time to answering the questions
listed below. Or, if you are unable to answer them in this format, perhaps my
research assistant or I could discuss these issues through a telephone
interview.
Please know that if you agree to participate, all of your feedback will be held
in confidence and no identifying information will be included in any reports
coming out of this project. If you are able to answer them by email, you can
address them in the space below and just send them directly back to me. If
you'd prefer to discuss these by phone, let me know and that can be arranged as
well.
