The Answer is:
A large thrombus consists of fibrin and platelets; a small plaque rupture results in a nonocclusive thrombus mainly of platelets. Thus, fibrinolytic therapy is effective in STEMI (high clot burden) but not NSTEMI (low clot burden) (Am J EM, 2/08).
