This study, reported in “NEJM,” focuses on the discharge procedures of hospitals and how to minimize readmissions.  Writing proper and complete discharge instructions is an important part of patient care.  Protect yourself and your patients!  Take the time to ensure that patients have a completed medication reconciliation form and that they understand their medications.
