The issue of the safety of metronidazole in pregnancy regularly comes up in EM practice. This has been the subject of some controversy in past years. In fact, there currently is no evidence to suggest that metronidazole places a pregnancy at risk at any stage. For some conditions, metronidazole is the only drug of choice and it is especially important to treat vaginal infections during pregnancy. Untreated infections such as bacterial vaginosis and trichomonas have been associated with miscarriages and preterm labor.
