Alameda Alliance for Health is a not-for-profit health plan committed to making a difference in the quality and access of health care services to the people of Alameda County. Alameda County Case Managers Monthly Management Meeting
MISSION: To enhance the knowledge of resources available to providers serving the HIV community and offer clients easy access to comprehensive services and keep them connected to primary care. HISTORY: Alameda County Case managers formed the Case Management meeting in 1990. This meeting consisted of only 8 to 10 nurses and social workers employed by the county who provided medical case management to individuals infected with HIV/AIDS, living in Alameda County. This group met monthly to discuss service coordination referrals and offer support. In 1991, the group was opened up to a new case manager who had just begun providing HIV psychosocial case management funded by Ryan White funds. By 1994, psychosocial case managers from various agencies were invited to attend. The meeting began to offer support when a client died, available resources as well opportunities network and names with faces. By the year 2000, the group had grown to include 25 service providers from all over the county that provided HIV services to individuals living in Alameda County. Providers who attend are able to follow up on referrals, receive updated treatment information and resources, and receive program updates, case consultation and support. Presenters have include doctors, attorneys, representatives from social security, and other social services in the community.

Alive & Well San Francisco, established in June 2004, is a local affiliate of Alive & Well AIDS Alternatives, the Los Angeles based nonprofit AIDS service organization. The mission of Bay Area Young Positives (BAY Positives) is to help young people (26 and under) living with HIV/AIDS live longer, happier, healthier, more productive, and quality-filled lives. The goals of BAY Positives are to work with individual HIV-positive young people to: (1) decrease our sense of isolation, (2) reduce self-destructive behaviors, and (3) advocate for ourselves in complicated HIV/AIDS care systems. La Clinica de La Raza
Before La Clínica de La Raza was established, low-income residents in the East Bay had few options available to them for affordable health care. As a result, many were forced to go to the Emergency Room for problems that could have been avoided with adequate care. In response to this gap in services, a group of concerned health practitioners, community activists and students came together in 1971 to establish a multi-service clinic based on the needs expressed by the community. Since that time, La Clínica has played an important role in the East Bay by offering low-cost quality health care services for multilingual and multicultural populations at 22 locations in three counties: Alameda, Contra Costa, and Solano counties, with many of our patients served in the City of Oakland. La Clínica's comprehensive services include: pediatrics, family medicine, women's health care, mental health services, dental and vision care, and health education. We offer these services regardless of people's ability to pay or insurance coverage. To most effectively serve the diverse community of the East Bay, La Clínica hires health practitioners who fluently speak Spanish, English, Chinese, as well as Hindi, Arabic, and Amharic. We also make a concerted effort to recruit doctors, nurses, health educators and other providers who come from the same cultures of our patients.
