Our church has yet to start a Parish Nurse program but I would think every
evaluation tool used may be individually tailored to your basic needs you
set up when you started the program. Such as number of Calls made to Shut
ins?
Hospitalizations/
Falls?
Needs survery results.
Congregation retension?

What is the purpose of your program? what activities do you involve your
congregation in? That is where a quick evaluation is easy to make.

I will struggle with you to make one if you would like.
