The parish nursing text "The Essential Parish Nurse" by Deborah L. Patterson,
includes a "Sample Service of Installation for a Parish Nurse," in Appendix C. I
have used this text, along with several others, as the basis on which to build
my ministry. It is an excellent resource.

Attached file is a copy of a service we use for our Parish Health
Partnership program. Our program provides a "parish nurse on-call" to
several area churches, we being a Lutheran ministry serving older
adults. I think the commissioning service still applies even though we
were commissioning lay volunteers who are the "health cabinets" in their
respective congregations along with the professional nurses on staff who
are the "on-call" Parish Nurses.
