You sound like you are on the right track by considering exercise for the osteoporosis.
I know you have probably considered the diabetes option and pathology neoplasms, endocrine disorders and chronic debilitating diseases. I am more in concerned with his weight loss in combination with the osteoporosis. I have the following questions for your considerations;
Does has demonstrate;
(a) Face ; boggy eyelids, puffy face and thick lips,
(b) hair? brittle, fall out, sparce look at the eyebrow,
(c) skin, thickened
(d) speech: slow? husky voice
(e) lethargy
(f) CNS drowsy
I would in combination treat him with some hormone therapy.
Is there any abdominal bloating? dull aching pain?
Upon eating is there any foods that help/ Exacerbate the pain? Consider any neoplasms
Consider any parasitic infestations? Does milk help him? Any heart burn etc?
What exploratory investigations has he had?
Any history of family predisposition of hyper/hypothroidism?
Maybe a more constitutional therapy such as minerals or celloids? He sound like he demonstrates malabsorption? Maybe a consideration of Ulmus rubra to start on any gut dysbiosis.
Let me know if you have considered this pathology. He will improve with exercise but what is his goal with the treatment? what does he want to do...?

I hadn't thought about Ashwaganda, we have sold it in our health food store for years, I might suggest it after the cleansing diet. I have been treating him primarily for osteoporosis nutritionally and with low level exercise with some success.
