When a patient is placed on trach collar, what is your standard practice? Do you
routinely deflate the cuff and remove the inner cannula while off the
ventilator? Please advise...

Yes we do. They are supposed to check all orders since the last 24 hr.
Chart Check. If there was not one done the night before, they should
check all orders since the last one. This includes making sure all tests
were ordered and completed, that all med orders match what is on the MAR
.

We do the 24 hr chart check but we just write in time date with nurse's name on
the order sheet to the point that we checked. Its completed at 12 midnight. No
additional form is filled out.
Same with my unit, however, the organization I used to work for had a form in
which you put the patient label and notated whether or not any missed orders
were found. It was a simple form with just a few columns for missed meds,
orders & procedures seperately so they could be tracked. This was initiated due
to a large amount of missed orders, not as a routine process.
We do end of shift chart checks and currently still do the 24 hour chart check
with a stamp stating chart checks. It is amazing what staff finds even with
shift and 24 hour chart checks.
I am seeing lots of emails about 24 hour chart checks. I have never
heard of this before. What is included in a chart check? Could someone
send me a list if you have one?
