During the day our case manager assists with the faxes
and bed placement/calls and copying of the chart.
Medical records can also assist with the copying if
needed. The nurse still assists and is responsible for
the transfer forms to be filled out and call report.
The case mgr. will also assist with transport team. On
weekends for emergent transfers it's the Charge nurse
arranging everything .
