Subject: Medicare Psychiatric Reduction

For Mental Health billers:

I have just discovered the 20% Psychiatric Reduction from Medicare
for an office in Kansas.  Arizona does not have such a reduction!  I
do not understand why Medicare, a Federal Program, reduces their
payment schedule in one part of the country and not in another.  I am
also amazed that they are doing this with claims with DXs between 290
and 319.  That pretty much covers all mental disorders.
How do you, the practitioners on this list, handle such a thing?  Do
you find other DX codes or procedure codes  that are still
appropriate to the situation, or do you just "take it on the chin"?

One doctor tells me that she will discuss meds with the patient and
then bill the code 90862.

I also see that the reduction will be phased out by 2014.

Frankly, I think this is one of the craziest things I have heard from
Medicare, and that is saying a lot!

Any input will be Greatly appreciated!

I have just started billing for an assistant surgeon in our practice using the
group tax ID# and his individual NPI as rendering provider. Is it possible to
have more than one billing and or service address associated with this tax ID#?
If we open another facility can we have payments sent to the place where they
were rendered even if it is not the billing address associated with that tax
ID#?

I think u should contact each Carriers providers relations dept and ask how each
works. My experience with the group TIN is usually each physician is paid under
the group not as individual physicians. So the billing address would be one,
however you can have multiple locations under the group. That's resolved with a
simple form that can be faxed. Some provider relations reps accept an email. You
know there's really no straight answer in the world of insurance;-) good luck!

If you have an EPM system, you would set up your payment to address different
than the serviced rendered address.   When you credential a provider with a
payer, you provide the both the payment address/lockbox address and the clinic
address(es).  Any additions or changes are submitted to the payers.

I have 4 clinics and a central billing office, as well as a lockbox.   Our
payers (most of them) keep it straight.  We set our Practice and each provider
this way.  Same TIN different clinic addresses and one lockbox address.

Thank you for your response. I neglected to mention that we do not participate
with any insurance provider. Does it make a difference? We are a small podiatric
surgical practice and don't submit electronically; all claims are sent on paper
with an op report attached.

If you see on your Medicare EOB, it puts the psych reduction to PR122, which can
be billed to the patient (PR=Pt resp). We bill it to the secondary insurance.

i have 5 clinics each has its own tax id# & npi, our billing office is another
location and is set up to receive all form of reimbursements, checks eob etc.

it is all a matter of how you fill your application to the insurance
company............

after everyones response you wrote again stating " you do not participate with
any insurance provider?????????"

so what is the point of your questions??????? maybe your statement is not clear!

participating means the dr & facility is registered with insurance company to be
paid, for services rendered to the insurance company members/patients!

"All claims are sent on paper with op report to who???????????????  where to
insurance company your provider participates with???????"

Ok, is anyone else very frustrated with this Obamacare insurance thing... with
carrying children until they're 26?

We had a 20 yr old pt come in for 4 visits this year and presented HIS employer
ins card UHC, for 2012 coverage. However, when his Dad came into pay his own
bill today he presented a Regence Oregon ins card, for his son. I asked how long
the policy had been in effect and he said "since birth." I told him that we also
had UHC on file, through pt's employer and Dad said "well let's just make that
primary." I told him we couldn't do that (by law) and it was up to
the insurances to decide who is primary & if claims need to be reprocessed.

I called both Regence & UHC and neither of them had any idea that there was
other insurance.

I had a patient a few mos ago. who came in and had no idea his Mom carried a
policy on him and he was married & had a policy through his wife. Ins determined
that because his Mom's policy had been in effect the longest her plan was
primary & wife's policy was secondary.

ARRRGGHHH!!!! Parents & children both have NO idea which plans are primary &
secondary (not just in this case)!!!!! No clue! WE (provider's offices) have to
inform them, once we figure it out, but it's getting them to inform us that's
the problem. Patients / Parents all want the "cheapest plan" to be the primary
insurance; regardless of what the laws state. 9 times out of 10, most of them
don't know which is primary or secondary. VERY FRUSTRATING!!! Because then
it's up to doc's offices to fix it. How stupid and time consuming.

So my question to the forum...in my above scenario for today, which do you think
is primary & secondary and why?

Also, is there some clarification on the web were I can get some documentation
on this? (i.e., birthday rule, etc.)

I would submit the claim to the insurance that has been in effect the longest.
 The three companies will have to determine who is primary.  I understand how
you feel; patients can really cause unnecessary stress sometimes.  Patients who
know next to nothing about their insurance coverage is and has always been a
problem. 

The insurance of the parent who's birthday comes first in the year is primary.
 You might try the insurance company's website for documentation of the
birthday rule.

FIRST & FOREMOST OBAMACARE HAS ABSOLUTELY NOTHING TO DO WITH THIS AGE OLD
SITUATION, FROM THE BEGINNING OF INSURANCE TIME.
(Patients who know next to nothing about their insurance coverage is and has
always been a problem.) it is our job to verify eligibility etc. you cant be
responsible for something no one told you anything about.  when you do find out
about it, then it is still your job to deal with refunds, takeback, resubmit
with proof of file time etc.


NOW: RULE OF THUMB
Patient can have as many insurance policies as they can afford
The insurance of the parent who's birthday comes first in the year is primary
Insurance companies will dispute & determine who is 1st, 2nd or 3rd based on
thier rules & policies
**NEVER RELY ON A PATIENT TO TELL YOU WHAT IS OR ISNT ABOUT AN INSURANCE
POLICY**

What would you recommend if you dont mind me asking?
My chiro is at a loss and dont know how else to bill..he is coding by what he is
doing.
We were getting paid no problem and all of a sudden they said we need modifiers
then they said no we dont .i cant get a straight answer from the insurance
company..
Uhc implemented a multiple reduction policy on the 97110..
