I run a medical billing company, and we currently are expecting to take on more clients. However, our operations have reached a processing capacity and we cannot expand anymore until we relocate. Therefore in the interim I am looking for a billing company/individual who is willing to take on these additional clients under our company banner. This arrangement should last at a minimum for about 6 months.

specifically targets medical billing. I may be needing a job soon.
My place is being sold to another company.
I have experience of over 12 years doing billing. I am located in Los
Angeles / SF Valley. I specialized in Radiology billing for last few
years but also worked for a regular physician for years, so am familiar

I am currently setting up the practice for a D&A Detox Clinic in Ohio. The owner has asked me to research salary info for an Office Manager and Receptionist.

Can anyone provide for me the hourly salary rate for an Office Manager and a Receptionist in the Dayton, OH area. He plans to pay him/her 1099 (he will not deduct taxes from salary, but will report it to IRS. Employee will be responsible for paying taxes directly to IRS).

Hello everyone -

I begin with an apology. It is not my intention to cause any inconvenience. I have been working on several things for CHAMPS so that you may go into the second half of the year with great marketing campaigns for your billing companies. But, I must put things aside for at least a week.

My oldest sister in Virginia died tonight 8pm EST. I will be traveling to Suffolk, Virginia perhaps tomorrow night or Thursday morning.

My sister was 50 years old. She had been in the hospital since June 15 of 2004. She had Crones Disease and suffered a great deal. Now she is in a better place.

I felt that I needed to send this message so that ifyou email me you would not think that I was ignoring you. It is truely not my intention.

I will check the board and my emails when I return. I will send out a message as soon as I get settled.

Has anybody used them?

Are they reliable?

How much does it cost per claim or is there a monthly fee? is there a
monthly minimums?

Do they have good customer service? I have been unable to get them on
the phone and there seems to be no answering machine there either.

Hello everyone:

I know that you have not heard from me in a while. I apologize for any incovenience that this may have caused.
This seems to be the year of misfortune for me.
Since my husband's stroke on May 3, my oldest sister's death on June 9 and the amputation of my mothers leg in April, I have been taking care of things at home and in Virginia. I know it sounds spooky, but, it's true.

I can gladly say that I am getting back on the right track and looking forward to moving ahead.

I am wiritng to let you know that if you have been part of the "Newbie Medical Billing Classes", they will resume on Wednesday, June 22 at 9pm EST. I am truely looking forward to meeting on the MSN IM Board again.
 
For those of you participating, don't forget, we were going through the Practice Management Registration and Scheduling proceses and reviewing scenarios. Please check your MSN email address for the worksheet if you have not done so.

If you wish to become a certified biller once the course is over, I have made arrangements with an Affiliate to administer the Exam. Cost can be discussed later.

Also, for those of you who requested notes from past classes, have no fear. I saved your emails and is in the process of responding to them. I may be a bit slow at the moment but, I did not forget you.

I am now able to concentrate more on CHAMPS as well. It's the summer, and we need to get our presentations in gear to go out into the health care community and introduce ourselves and meet the professionals and grow our businesses.

Recently reviewed the answer to your initial request for info below. I graduated over over two years ago and since that time I have tried unsuccessfully to obtain employment in the field.
I followed all the guidelines and suggestions you received as replies and in the two years I was unable to garner any interviews. Not a singe one!
 
I did the homework, laid my ground work and although I have many of the strong points (some of which are listed in the reply) as well as being a former Medical Assistant, Medical Secretary and Medical Office Manager again, not one single interview.
 
Needless to say, I feel that the time I spent training was time wasted. I still work in the medical field but as an Admin. Asst.  ---- just what I wanted to get away from.

There are various types of billing and payment mechanisms. There are
also health policy issues that have nothing to do with payments- more
so compliance issues.

When medical billing is done, there are two components: Technical and
Professional. Technical billing is done for a facility - hospital,
skilled nursing, long term care, rehabilitation. Professional billing
is done for a physician or practitioner. A practitioner is someone
who has not obtained a medical degree but is able to see patients and
prescribe medication - nurse practitioner, physical therapist, speech
therapist, licensed clinical social worker...

There are different payment schedules for facility and professional
billing. Moreover, the schedules vary based on the payor type -
managed care, workmen's compensation, grants, Medicare and Medicaid.

Physicians are paid based on something called Relative Value Based
Units: this is based on three things- Cost it takes to deliver care,
Malpractice insurance and Production hours.

If you have not begun to do so, now is the time for you to start your
homework engine.

First. To get a feeling for billing, visit the following websites.
Veritus Medicare (Part A) HGSAdministrators (Part B). These companies
have billing policies and manuals that are the Bibles for the medical
billing world.

The Centers for Medicare and Medicaid Services (another great
website), is the federal government agency that sets the mandates for
healthcare in the US. This organization contracts with private
insurance carriers to administer their Medicare and Medicaid programs
and process claims for people with these types of public coverage.
The private companies are known as Fiscal Intermediaries (FI). They
are organized based on the programs they administer: Technical - Part
A and Professional - Part B. They are also broken down by state.
Pennsylvania's FI is Veritus Medicare for Part A and HGSA is for PArt
B. Durable Medical Equipment also has a Fiscal Intermediary branched
out by the states.
Second. Decide if you want to pursue technical or professional
billing. Make a decision as to what specialty to pursue. It would be
easier if you concentrated on one specialty to get a feel for that
branch of medicine, ie - (professional) diagnostic imaging,
laboratory, neurology, chiropractic, physical therapy, dental.
(facility) hospitals - acute, long term care, skilled nursing,
hospice, behavioral health.

Third. Procedure coding is one of the primary keys to providers
getting paid (the other is documentation -another story). These codes
are always 5 digits. HCPCS and CPTs are the procedures (how the
doctors service the patients). These codes are used for physician and
ambulatory services only.
Revenue codes are used for facilities. They are always 3 digits and
fall into a certain classification.

ICD-9 is the diagnosis or disease. These two coding mechanisms must
be on the claim.

For example, I had an office visit with a pulmonary doctor -

Procedure (99354 - office visit) Diagnosis (135 - sarcoidosis) Have
some fun, research these two codes and see what you come up with.

Fourth. Healthcare medical and payment policy changes everyday. You
need to stay abreast of what's happening in the world of healthcare.
Join several professional organization enewsletters and Listservs.
Many do not cost anything.

Visit www.tonhealthcare.org, go to the site map and click on industry
links to discover a few of these organizations.

Fifth. You have a lot of competition. If you do decide to pursue
this, be prepared. Do your homework. A physician will not turn their
business over to just anyone. The skill set required to do this
includes, excellent organization abilities, detail oriented, multi-
tasked, strong interpersonal and communication (oral and written)
skills and, the ability to be team oriented and proactive. It has to
be a win-win situation; the physician and office administrator has to
trust that you will get the job done.

Sixth. Join a professional organization that is geared toward what
you want to do. Some of them can be expensive. If you can't afford to
join, see if they have a listserv or enewsletter (ezine).

Seventh: You need to decide what type of services you will offer.
Some companies offer claims submission only. Some offer claims
submission, A/R clean-up and Interim services.

Eight: Decide how you are going to charge for your services.

Ninth: Decide how the provider will get the information to your
office.

Tenth: Create Standard Operating Procedures for doing business
internally and externally

Eleventh: Decide on a marketing campaign. Vary the campaign

Does anyone know if there is specific documentation anywhere to let
physicians know that the medical record visits diagnosis must match the
encounter/bill sent to the insurance company? I have a physician who
basically says when he sees a pt for a f/u in the office.. and puts in
the chart f/u for say htn, dm, ras and then makes his notes.. without
at the end of his documentation either doing an imp or A with the dx..
that the office girl should then look at the chart.. see what they were
being f/u for and base her dx for the encounter on that and in any
order.  or .. he is having her simply pick 3-4 off of the encounter
which has old dx's simply for a reminder of what we have seen them for
in the past.. any suggestions.. i basically told him what i've been
told at seminars/conferences about the importance of documentation
being specific and matching the billing codes.. but he only brushes it
off saying people who do these conferences are only telling you their
opinion and that he has read the federal register which says nothing
about that being mandatory. unless i show him documentation he refuses
to change and tell the girls the dx's for the visits. If there is
documentation could someone send it to me.. and if i'm simply wrong in
what i'm telling him.. let me know that too!! thanks!!

I will research this tonight.  As an auditor I know that the diagnoses must match the reason for being seen as documented in the note for that day's service.  For example, if a patient comes in for a joint injection, but his last encounter was for diabetes mell, well that would be incorrect.  The Federal Register is not the only place that a physician/coder/biller/manager should look for information.  I will find the documentation. I believe it is in the ICD-9 guidelines.
 
Try approaching  him explaining the impact on the bottom line.  Quicker payments, and less labor spent on denials.

Would it be possible to enlist the aid of one of those other physicians?  If he's like my boss, he'll only listen to degreed males descended from southern Asia!  LOL!  I often have to enlist the aid of his partner, accountant, etc to convince him about things, becuase in his mind there's no way an "uneducated" white woman could possible know what she's talking about!  Sometimes you have to enlist the aid of someone on self-preceived level!

thanks Leah.. that is actually exactly what i did the next day!! 4 of his partners sat him down to not only discuss the total unprofessional behavior he exibited in front of the front office but also the fact that once again.. he was totally wrong and needed to work on changing the way he did things!! thanks everyone for all your help!!

I was just terminated from the employer from h_ll. The doctor that I work with sold his business to this other doctor a couple of years ago. This other doctor and his wife has been committing insurance fraud, the wife,who is our office manager, has embessled (?) money from these offices. They  both are violating Hippa. Where can I find help to put these criminals behind bars?
Can anyone help me?
