The other most common factor that makes a dictation difficult is the
terminology. This does not always mean medical terminology, although
that can be a stumbling block when you start doing superspecialties.
(It is like being the member of a secret club with its own secret
passwords and code language. But with the wide range of wordbooks,
and specialty books available today, searching for specialty or even
superspecialty words is no longer the task it was even five years
back.) However, this difficulty often comes when the dictation has
references to culture or lifestyle. The only way to overcome this is
to familiarize oneself with the basic reference points of geography,
history, economics, contemporary culture, etc., particular to where
the dictator is coming from. The Internet is a blessing in this
area, since it makes available to you local flavors at the click of a
mouse. However, one would be well advised to keep in mind that the
Internet is an unregulated medium and there is no way to be sure that
what you find on the net is correct unless you are using reliable
portals or websites.
The bottomline is that every component in the field of healthcare
documentation is human, starting from the patient to the physician to
the MT to the office manager who okays the transcript. The process
is one of communication, the contents of which have to do with the
human condition, and the tools, language, spoken and written. The
possibilities are infinite but all of it have to do with the issues
basic to us as a human race. Once we have that in place in our frame
of reference, the task of transcription not only becomes easier but
also vastly enjoyable and illuminating.
The journey into the world of transcription is an exciting one. The
joys and rewards of the journey do not lie at the end of it, they lie
along the way. If you keep your eyes and your ears open, you will
find that every passing day brings you gifts, be they in the form of
new knowledge or words or usage, or in the form of feeling fulfilled
at being part of bringing quality healthcare to your kind.

1) AHIMA to Feds: Need Final Security Rule Now
The American Health Information Management Association (AHIMA) sent
a letter last week to the Departments of Health and Human Services
(HHS) and Defense (DOD), raising serious concerns with two events
which occurred at the end of 2002 that highlight a need for the
publication of the final HIPAA security rule. The letter, addressed
to HHS Sec. Tommy Thompson and DOD Sec. Donald Rumsfeld, points out
HHS' failure to issue a final notice for HIPAA Security
regulations, as anticipated on December 27 and the theft on
December 14 of more than 500,000 health records by a DOD
contractor. A $100,000 reward is being offered for information that
helps lead to the arrest and conviction of the perpetrators who
stole the computers from the Phoenix offices of TriWest Healthcare
Alliance, part of DOD's TriCare system. A Defense health official
said the theft poses no threat to the Composite Health Care System
II, DOD's pilot computerized medical system.

2) Max India MD Vivek Jetley puts in his papers
Vivek Jetley, the man who strategised and implemented Max group's
foray into the new economy businesses such as insurance, healthcare
and IT as the MD of the group's holding company Max India, is finally
parting ways with the group. He is learnt to have already put in his
papers. Significantly, Mr Jetley's departure comes at a time when Max
announced its intention to scale down its operation in IT business
recently.
He is credited with finalising the strategic partners for all its new
economy businesses — New York Life Insurance for the insurance
business, MindCrossing and Helathscribe for its IT and medical
transcription businesses and Harvard Medical as a technical partner
for the healthcare business.

3) Judge orders tax protester to cease marketing his services
A man who represents taxpayers contending they are not required to
pay income taxes must stop inciting tax evasion and turn over
copies of his clients' returns, a federal judge ruled. A
Queensbury, N.Y.-based organization called "We the People"
advocates against filing income taxes.
Larken Rose, who says he has informed state and federal officials
every year since 1998 that he will not be paying income taxes on
his medical transcription business in suburban Philadelphia,
estimates that at least 10,000 people do not file income taxes for
the same reason.
He argues that tax codes do not specifically mention domestically
earned wages as taxable income, and that regulations in the tax
codes state that income excluded from mention in the law meets the
definition of exempt income. The language exempting him and many
other Americans from paying domestic income taxes "isn't a matter
of a loophole," but was the intent of the federal tax codes when
written 80 years ago, said Rose, who is 34.

4) Apollo Hospitals sign up Lincoln Hospital
The Apollo Hospitals group has signed a multi-million dollar contract
for medical business process outsourcing (BPO) with Lincoln Hospital
of New York. The chairman of Apollo Hospitals, Prathap C Reddy, told
PTI that the group is entering in a big way the medical BPO segment
wherein clients would be given services on coding and billing
processes.
The group has already set up a development centre at its Hyderabad
premises and intends to set up IT-enabled service centers at all of
its hospital premises within the country. The group would also use
Internet-based technology and telemedicine for treating patients in
far flung areas.

5) Data-entry scam could cross Rs 100 cr
The arrests of businessmen in Vadodara in the data-entry racket could
well be just the tip of the iceberg of a much larger scam involving
hundreds of people and many more companies.
Gujarat Electronics and Software Industries Association (Gesia)
president S N Khemka believes this scam could cross Rs 100 crore by
the time the details are unearthed. Proprietors of two firms, Nitin
and Nikesh Exports Pvt Ltd (NNEPL) and Melfa Infosys, have been
arrested for allegedly not paying several people who took data-entry
jobs from them.
The scam has hit at a time when Gujarat is seen by Nasscom as one of
the hottest destinations in business process outsourcing (BPO). Says
Avinash Bhandari of Mangalam Infotech, a medical transcription
firm, "BPO is the largest growing segment with 80% of the work
related to data entry. When such large volumes are being transacted,
some unscrupulous people are bound to take advantage."
