Hi there,
I am SAM, an Iraqi refugee living in Lebanon at the moment; I have spent the
last 10 years of my life as a refugee registered with the UNHCR in Beirut. The
last 4 years, I have spent as an activist for peace and human rights (especially
refugees and asylum seekers) on the Internet; I'm also books author and ebooks
publisher. I have launched many campaigns to improve our situation as refugees
in Lebanon and hopefully bring more understanding to our problems worldwide. I
helped make many changes and improvements at the UNHCR office in Beirut; I used
the Internet as the field for my activities (you can read more about that in my
free ebook 'MY CAMPAIGNS'). All my ebooks are free and could be download from my

This is my newest campaign, it's about the illegal and humiliating actions of
the UNHCR, who using photos of refugees as banners and human-buttons to collect
money. This is an abuse of the dignity and humanity of the refugees and must
stop immediately and a clear public apology present by The United Nations High

Understanding the psychosocial barriers to effective diabetes therapy>>

The scale of the problem that diabetes poses to world health is still widely under recognized. It is estimated that if the current trends continue the numbers of people with diabetes will almost double from 194 million people in 2003 to 333 million by the year 2025. Diabetes is already the most costly health care problem all over the world. Epidemiologic studies in Iran indicated the number of people suffering from Diabetes type 2 is about 2 million, it is estimated this number will be 3,200,000 by the year 2020.This number do not consist the number of people who are not aware of the underlying condition .>>
Beyond the routine advancements for coping with a variety of recognized diabetes complications including retinopathy, nephropathy, neuropathy and cardiovascular manifestations, recent studies highlight the importance of psycho-social factors in diabetes management. Research shows that psychological co-morbidity is prevalent in people with diabetes. As a result well being, self care and glycaemic control are adversely affected. Depression is common in people with diabetes and unfortunately under diagnosed in many cases. A person with a major depressive disorder suffers from a depressed mood and or a loss of interest or pleasure in daily activities for a prolonged period of time. This disorder is characterized by the presence of a number of symptoms including significant weight loss or weight gain, insomnia or hypersomnia, feeling of worthlessness or excessive or inappropriate guilt and recurrent thoughts of suicide. Recent studies including the Diabetes Attitudes, Wishes and Needs (DAWN) in US and many European countries indicate that in general psychosocial support is under-resourced and inadequate in both adults and children with diabetes. In order to minimize the risk of diabetes complications we need to initiate:>>
Qualified communication for improving quality of life in diabetic patients. Quality of life is increasingly used as a factor in the evaluation of the quality of care. In diabetes care self report questionnaires are employed in large scale inventory research into the human experience of people living and condition. While this approach is both logical and positive it can not replace effective communication between people as a means of understanding the individual needs for each person. Health carers who are committed to providing quality diabetes care must be prepared to spend more time mediating intense emotional interactions with people who have the condition. To achieve this health carers require improved communication skills and adequate psychological knowledge.>>
Diabetes education >>
To improve health outcomes and ensure the total well-being of people with the condition it is important to reach the person beyond the laboratory results and blood glucose reports. Diabetes education can make an important and positive impact ina number of people with Type 2 diabetes. At the beginning of Insulin therapy an exaggerated fear of injections, a lack of information and understanding about insulin and some widely held misconceptions about the functioning of the hormone often lead to psychological insulin resistance. The anxieties and resistance to medication which often accompany Insulin therapy can be conquered with the support of a diabetes education program which reassures people that the need for injection of insulin dose not represent a personal failure, helps them to learn about how to prevent and manage hypoglycaemia accompanies them through the injection process. As a whole diabetes education is effective in improving both metabolic outcomes and quality of life.>>

People with diabetes or those who have a close personal relationship with a person with diabetes know how strongly the condition impacts on every-day life. Studies demonstrate that diabetics have significantly more psychological problems than the background population. Rates of depression have been seen in people with diabetes which are 2-4 times higher than those in the general population. The psychiatric problems may complicate and impact negatively on diabetes management and outcomes. These will require medical and behavioral assessment and treatment .A team consisting of a general practitioner, a diabetes nurse or a specialist diabetes doctor is needed to help people adjusting to life with diabetes.>>
Keep people at the center of care>>
The establishment of a practice which puts the person at the center of care will require a change in the attitudes and beliefs of health professionals and people with diabetes. A service which places the person with diabetes at the center of care will undoubtedly demand the adoption of this philosophy by the organizations responsible for the delivery of diabetes care. A recently diagnosed person with diabetes needs a level of confidence in order to take a series of decisions regarding the management of his or her condition. In order to this reassurance should be offered by the health carers.>>
The expert people with diabetes could also be considered an important resource for the health service as effective health campaigners and members of health care bodies.>>
In a world in which health care system are failing people with chronic health conditions we need a new paradigm in which people with diabetes become their own health care providers with health care professionals playing a supporting role. In diabetes care the principal objective is effective therapy for people with type 2 diabetes involves lifestyle changes and poly-pharmacy targeting levels of blood glucose, blood pressure and blood fat. Barriers to effective diabetes therapy have been identified within the organization of health care and in the interaction between health care providers and people with diabetes. >>

The UNICEF/UNDP/World Bank/WHO Special Programme for Research and Training in Tropical Diseases (TDR) invites applications for the award of Collaborative Research Grants (CRGs) on a variety of social research topics defined by the TDR Steering Committee on Strategic Social, Economic, and Behavioural Research (SEB):
Topic 1: Understanding and remedying access barriers to
established measures of prevention, therapy and information
Topic 2: Understanding fundamental social issues related to stigma and adherence to treatment and prevention and improving adherence, with a focus on mass drug administration and other prevention schemes
Topic 3: Elucidating the dynamics of human resources in infectious disease control programs
Topic 4: Elucidating transaction costs in infectious disease control programmes
Topic 5: Understanding and addressing child vulnerability and resilience, with a focus on infectious diseases among children in conflict situations

Full proposals must be submitted on TDR forms, with research clearances, ethical review board approval, consent forms and research instruments, in the original language and in English translation, following the guidelines provided below and on our website

The SEB Steering Committee holds one meeting per year during which it updates its workplan, reviews new research proposals and reports, and monitors overall progress of the Committee. The next meeting is scheduled for May 2006.
The Committee invites individual research projects with budgets normally not to exceed $35,000-40,000 per year for a period of preferably one to a maximum of two years. The committee is particularly interested in supporting projects that involve South-South and South-North partnerships, between institutions and individuals that will contribute to research capacity building for social science research in disease endemic countries. For group projects and, in exceptional cases individual projects, the Commitee will consider projects requiring greater
levels of funding.
Projects are funded on an annual basis and renewal of funding is based on the success of progress and delivery of a full technical and financial report.
How to apply
Detailed Proposal Development Guidelines for SEB research are available on the TDR website at http://www.who.int/tdr/topics/social-research/guidelines.htm and should be closely followed while developing the proposal. Collaborative Research Grant (CRG) application forms and instructions are available from TDR at http://www.who.int/tdr/grants/grants/collgrant.htm.
Applicants are invited to be succinct and not exceed 10 pages for the full proposal/protocol, excluding instruments If you are interested in submitting a proposal to the SEB Steering Committee, an early letter-of-intent can be submitted to the SEB secretariat to request feedback before developing a full proposal. Applications will be reviewed by the Steering Committee each
May.
Applications can be submitted in English and French only. The steering committee encourages research teams from other major language areas (e.g., Spanish, Lusophone, or Arabic-speaking countries) to apply in English or French. The Steering Committee strives to support the broadest possible range of countries, institutions and diseases under the TDR mandate.
In your application, please indicate the topic and topic number you are applying for.
After approval of a funding recommendation by the Director of the TDR Programme, the Manager of the Steering Committee will guide the proposed Principal Investigator with respect to further steps needed to complete the protocol development and application process.

The recent spread of highly pathogenic strains of avian influenza has highlighted the threat posed by pandemic influenza. In the early phases of a pandemic, the only treatment available would be neuraminidase inhibitors, which many countries are considering stockpiling for pandemic use. We estimate the effect on hospitalization rates of using different antiviral stockpile sizes to treat infection. We estimate that stockpiles that cover 20%25% of the population would be sufficient to treat most of the clinical cases and could lead to 50% to 77% reductions in hospitalizations. Substantial reductions in hospitalization could be achieved with smaller antiviral stockpiles if drugs are reserved for persons at high risk.
Cost-Benefit of Stockpiling Drugs for Influenza Pandemic Emerging Infectious Diseases 2005 Aug;11(8):1280-1282.
We analyzed strategies for the use of stockpiled antiviral drugs in the context of a future influenza pandemic and estimated cost-benefit ratios. Current stockpiling of oseltamivir appears to be cost-saving to the economy under several treatment strategies, including therapeutic treatment of patients and postexposure prophylactic treatment of patients' close contacts.
