If your carbs are low, all you have left is either high protein or high fat.
Suggesting a plan is a bit like suggesting a religion to someone. With that
in mind, I would suggest you pick up "Dr. Atkins New Diet Revolution."
Given that you say you have a health problem, which could be anything, I
hesitate to suggest more that this.
For example, in my case I had high triglycerides around 225. By changing to
a high fat diet I brought the triglycerides down to 105. It is unfortunate
that you cannot eat fat.
