Packet in an email this weekend. All forms are there except the Accommodations
form which will follow in a few days. Please send all forms together with your
checks to Mary-Lee Banek at 206 Hamlet Drive Burnsville NC 28714 ASAP and before
deadline of Oct 18th.
If you are attending True Self Retreat, additional information and forms will be
sent out shortly.
Please be sure to include all the following forms with your checks made out to
the correct person:
1 Registration Form (your check made out to Irmansyah Effendi)
2 Release Consent Form
3 Master Yoga Form with picture attached if taking MY 1-1 for first time
(picture can be a copy of your driver's license)
4 Shing Chi 9 or 10 donation (your check made out to Irmansyah Effendi)
5 Meals and Travel Form (your checks made out to Dianna Lee)
6 Accommodations Form (your check made out to Heart Sanctuary)
7 Any additional Forms for True Self Retreat
(Coordinators I will be sending you additional information when it is received
from Home Office.)
