Procedure aids women suffering incontinence
Marvin Dietrich, Gary Newman and David Forest
Special for The Republic

QUESTION: My pelvic floor is sagging, causing me to have incontinence
problems. Are there any new options for women like me?
ANSWER: We were the first in the Valley to perform a new procedure to
secure a sagging pelvic floor. The posterior intravaginal slingplasty
procedure is a new, minimally invasive approach that applies a mesh
tape to the upper part of the vagina. A piece of surgical mesh tape
is threaded at the top of the vagina and through the pelvic side
walls to support the top of the vagina. Then a fibrous-tissue
reaction occurs around the tape, which strengthens and replaces
weakened pelvic ligaments and suspends the top of the vagina in its
normal position.
The procedure, developed and studied in Australia and Europe, has had
success rates reported to be approximately 90 percent.
If pelvic-floor muscles become weakened, these organs are no longer
supported, which can result in incontinence or other symptoms that
may warrant corrective surgery. The posterior IVS can reverse these
symptoms. This is important because half of all women experience
incontinence at some point in their lives and about one-third develop
a regular problem. After age 60, incontinence is twice as common
among women than it is among men.
Another new option is Medtronic InterStim Therapy that helps control
urinary problems through an implanted device. Mild, electrical
impulses are sent to the sacral nerves below the base of the spinal
cord, that control the bladder, sphincter and pelvic-floor muscles.
This "pacemaker for the bladder" is indicated for the treatment of
urinary retention and the symptoms of overactive bladder, including
urge-incontinence and significant symptoms of urgency frequency,
alone or in combination, in patients who have failed or could not
tolerate more conservative treatments.
In fact, this therapy is being considered by the FDA as a potential
treatment for interstitial cystitus when other treatments have
failed. IC is a condition characterized by chronic pain of the
bladder and pelvic region with an urgent need to urinate.
Electrical stimulation through InterStim Therapy is distributed
through a lead implanted adjacent to the sacral nerves. This lead is
connected to a neurotransmitter the size of a stopwatch that is
typically implanted under the skin of the upper buttocks, below the
beltline, or in the abdomen. The electrical stimulation ultimately
strengthens the pelvic floor and bladder muscles.
In clinical studies, InterStim Therapy, which is reversible, has been
shown to successfully treat certain bladder-control problems in many
patients who had failed or could not tolerate other treatments.
InterStim Therapy offers hope for women who have tried medication,
biofeedback and pelvic-floor exercises without symptom relief, but
without undergoing a non-reversible procedure such as bladder
augmentations, removals or urinary diversions.
These and other procedures will be discussed at "Female Bladder
Problems?" a free forum on Feb. 6, that you may want to attend. Call
the Sun HealthLine for more information or to register. This could be
the first step in regaining your physical health, quality of life and
self-esteem.
Drs. Newman and Forest are with Sun Health Obstetrics and Gynecology.
Dr. Dietrich is on the medical staffs of Sun Health Boswell and Del
E. Webb Memorial hospitals.

if you went to see your family doctor he should have
referred you to a urologist. if he did't look in the
yellowpages for one in your area. as far as disposable
briefs (diapers) go to your local drug store and look
for incontinence products. or you can buy them on line
at a number of sites just do a web search by typing in
adult diapers. the site I buy them from is
<www.hdis.com> they also have a toll free phone in the
USA(1-800-269-4663)
you might want to do some research on incontinence.
Hope this helps and good luck.

My number 1 advice: See another Doctor. At 16 incontinence is not a
normal and is not just a "wear a diaper" situation. Many tests should
be done and specialists consulted to find the source of the problem.

May i ask why you are fighting it and worring about it so much.
Wear diapers or a pad and i might want to choose some new friends if there
making fun of you.
But dont worry about it try and help your self as much as possible but dont
stress over it and there should be no reason you cant make freinds or hold
up a full time job i have pently of friends and hold a job and wear diapers.
Dont let this get you down every one has a medical problem at one time or
another.

Hi Emma
Yes i would love to get in touch i relied to your e mail add but not sure it got through Please contact asap for the time being on my second e mail add of twizzell@...
Greetings,
I come from a family that has a few incontinence issues: namely my
mother, sister, and an uncle or two. Never had to wear diapers myself
growing up, and nobody in my family ever did either, even though a
few should perhaps be in them from time to time. Too much pride.
With myself I started developing problems with prostatism, which
makes it very difficult to void urine, always left in pain with a
spasming bladder, at the age of 21. Sometimes I had to go to the
washroom every 5 minutes or on the hour it would seem. Just to attend
lectures and not sit on the toilet all day, I had to start wearing
pads and diapers. I did this at first while complaining to my doctor
at university, while not complaining to them directly about
incontinence issues. We went through tests, ultrasounds, etcetera,
and nothing could be done about it. I was offered surgery, but
everyone including myself preferred to wait atleast a year to see how
long it would last.
Today, about 7 years later, I now have prostatism here and there, but
it is often quite rare. But my bladder has suffered a lot over the
years, and has developed some odd habits, wanting to void every hour
or more - 8-30 times a day. So now I mostly have urge incontinence,
needing to go to the washroom all of a sudden . . . That is my
bladder can completely empty, but it needs to do so very often.
I'm working on better habits to control my bladder. This can be
difficult since I do a lot of construction work and am usually
physically exhausted. But on the plus side, I ussually work outdoors,
and can urinate almost anywhere like a barbarian, feeling free to
pull my pants down and go almost anywhere.
I still wear diapers though on occasion, like when I'm at home just
to make life more convenient. Because I sometimes wet the bed now 1-2
times a week, which I never did before. And I feel that I'd rather
use them in public, like when out shopping and stuff, when I can't
just pull my pants down and go. So in a lot of ways it is quite
confining. Trying to balance this problem between my home life and
work life can be difficult. But I do a lot of yoga in a studio, kegel
exercises, etcetera, and this really has helped.
Socially this problem, however, is quite aggravating. I've lived in
Toronto, Ontario, for 4 years now, and have not made any friends. I
just have 3 roomates who know I wear diapers and use the washroom a
lot, and sometimes snicker politely behind my back. Nothing is ever
said . . . Anyhow, I've learned how to be alone a lot, and to enjoy a
lot of privacy. I've had to.
Anyhow, would like to meet and talk with others who have the same
issues.
Sincerely,
Matt G

I've never thought catheterization was particularly painful,
either, even before I started doing it in-and-out, several
times daily some years ago. And Foleys actually are
reasonably comfortable after a very short time to adjust to
the sensation. Catheterization is easy to do.

Hmmmmm...... 12 MILIMETERS long? I suspect this is a major
typo..... (grin) What urethral surgery did you have and how
long will you be cathed?

Good group, Nick. Perhaps some of those who are new to the
need for various urinary appliances will be helped.

Merry X-mas
Hi everyone. Last Wednesday morning, my doctor installed a catheter
because the sheath version I had been using was no longer practical
following uretheral surgery. It wasn't too difficult nor painful. I
just laid down on her couch, lowered my jeans and underwear, moving
aside the diaper, and she injected a saline solution down the tip of
my (circumcised) penis. Then the 12mm long tube was gently inserted,
and I've been cathed up successfully, and mostly comfortably, since!
More about this, and pictures of me so "cathed up" are on my MSN

Best wishes, everyone

Hi all,
I'm new here and can't tell how serious this group is but I wanted
to share my experience in the hope that it can help others.
Like many menopausal women, I suffer(ed) from stress and urge
incontinence. When I would cough or sneeze or even laugh I would
leak urine (sometimes a little, sometimes a lot) all over myself. I
had this after I gave birth too but it went away so I thought it
would go away this time too...but it didn't. After several years
and a lot of pads and dry cleaning and embarassing moments, I
finally talked to my dr about it. He basically told me that my only
options was surgery. Having had a very bad surgical experience in
the past, I wasn't about to have another operation without some
serious research.
In doing my research I came across a device which claimed to be able
to help me strengthen my pc muscles to the point that I wouldn't
need the surgery. I was skeptical but willing to try since the
surgery was such an unpleasant concept. I was a bit hesitant at
first because of the expense ($80) but I went ahead and tried it
since I realized that pads and/or surgery would eventually cost me
way more than that. IT WORKED!! After exercising for about 3.5
weeks, I stopped leaking completely. I can laugh and even go
running again. It's so, so wonderful that I had to share for anyone
having the same problem. The device is called the Kegelmaster
(didn't really like the name but hey, it worked). It's available
all over but I found the best deal at
http://www.informedchoiceproducts.com
I really, really recommend it for any woman suffering from stress or
urge incontinence. I'm happy to answer questions about my
experience using it so feel free to email me. I hope this email
helps at least one other person, then I'll feel I've done a good
thing.
Blessings to you all,
Tip
I mix cloth and disposables to keep the costs of incontinence down.
When I am at home and have no plans to leave the house I try to wear
cloth, but disposables are just easier for when I am busy (which is
most of the time.)
