Phase III of the Huntington's Disease Memorial Quilt made it's grand entrance and unveiling at the HDSA National Convention in June, 2006! As most people know, all three of the Huntington's Disease Memorial Quilts are available, on a first-come-first-get basis for any HD awareness or fund raising event.

Requests for Loan of a Quilt
Since all of the Quilts are maintained in the home of HDSF Committee members Dave & Sue Hodgson in Sandwich, Illinois all instructions have been changed to reflect future requests for the loan of a Quilt for your HD event need to be submitted to Sue Hodgson. Sue's email is sue_angels@.... It is preferred that each person requesting a Quilt fill out the HD Memorial Quilt Application for Loan however, as a minimum, your request should be emailed to Sue.

Shipping Cost
Since the Quilts are stored at Dave & Sue Hodgson home if they aren't out on loan at another location, the Quilt you requested will be shipped Illinois. Since donations haven't been collected from locations displaying the quilt in the past, there are no funds in the HDSF Scholarship account to cover shipping to a requesting location. In 2005 the HDSF paid for the shipping which reduced the monies we had in the Fund for convention scholarships. The Quilts travels to HD Events approximately 12 times each calendar year so hurt our 2005 scholarships.
In May 2006 the HDSF found we need to start having the requesting location to pre-pay the cost of shipping any of the Quilts for their HD event IF the quilt is being shipped from Illinois. Rather then having to get a quote from UPS each time a location requests a the loan of a Quilt, the HDSF has come up with an
approximate UPS cost to ship each quilt [based on prior costs], depending on its weight and size:
Phase I -------------$45.00
Phase II ----------- $35.00
Phase III ---------- $35.00
Some locations may be closer to Illinois, however many are much further away. The HDSF will deposit your pre-paid shipping charge. Any money not used to ship the quilt to your location will go towards the 2007 convention scholarship fund.

If the group requesting the quilt wants to display more then one quilt, the shipping charges would be cumulative since they are shipped separately. For example, to ship Phase I and II, the UPS cost to ship to you would be $80.00. Each Quilt is shipped via UPS in a canvas duffle bag along with the binder holding the bio on each square and the guestbook.
Each is insured for $1,000.00. Although the each Quilt is priceless and could NOT be replaced, should a claim ever need to be placed the insurance reimbursement would go to the HDSF convention scholarship fund.

For those that would like to charge the cost to ship a Quilt for your event, once your reservation has been confirmed by Dave Hodgson, Pay Pal has been added to the HD Memorial Quilt Loan Application link: http://huntingtondisease.tripod.com/hdmemorialquilt/id1.html Once Dave, the HDSF Treasurer, receives a notice from PayPal indicating you have pre-paid the shipping, the Quilt will be shipped.
