Nelson, I live in northern Minnesota, north of Duluth, and I go to Dr. Keith Henry in Minneapolis, who I have high regard for.  I am sure you have heard of him, maybe even met him.  Nelson, you said "There is a study that recently showed that those who have activity to Aptivus (Tipranavir) and Prezista can take Aptivus first and will still be sensitive to Prezista if the Aptivus fails. "  I am concerned about how this works...If the Aptivus fails if I take that one first, then won't that leave room for the others to fail before we find out it has failed? As you know, I am very concerned with doing this right this time.

What would you think if I took TMC 114 AND TMC125 plus MK518, plus Truvada...wouldn't that be a pretty strong combo all together (if the doctor feels it would be safe)?  Maybe I am just too scared of failure these days.

I want to personally thank you. I have emailed you previously and always get an informative response and I want you to know I appreciate this very much.  You really deserve an award for your work.

Robert
Dear Robert

I believe in trying to save options for the future while trying your best to attain undetectable viral load now with at least two active agents plus nukes.  Three active agents would probably be best but no one is sure. There are no data on the TMC 125 + Prezista+ MK 518 combo, but we know that there are no bad interactions between those three that could lower medication blood levels that can cause virologic failure.

Trying Aptivus before Prezista may preserve Prezista for later use in case that you need another combo in the future.  The drug pipeline will dry up in the next two years again.  I feel we should hope for the best now but prepare for failure in case. The disadvantages of Aptivus, however, is that you cannot take it with TMC 125 since it lowers TMC 125's blood levels. Aptivus also requires more pills and two Norvir a day. I am sure Dr Henry is aware of the recently presented data in Barbados on Aptivus and he can discuss it with you.

I think you would have a great change at having undetectable viral load with MK 518+Prezista+ TMC 125. We are hoping it is a sustainable response long term, though.

Thanks for the compliment. I am just a guy trying to survive HIV resistance and have put all my obsessive compulsive behavior to use in learning all I can about new treatments by going to many conferences and reading emails that are sent by many sources. I will be going to the largest HIV research conference in the world
Regards,
