i really hope i get a response as this has become serious. i am a 21
yr old female who had a lil girl in may 2006. about 2 months later i
started experiencing excessive gas. no matter what i put in my mouth i
build up gas. some nights it gets so bad that i literally have to stay
in the bathroom for hours. i get bloated and crampy but not all of the
time. EVERYDAY however i have excessive gas. its one right after
another. ive always had a constipation problem but ive never had a gas
problem. i did have an episitomy done on me when i gave birth to my
daughter and i was told by a gyn that it might cause loose stools and
gas but i dont have loose stools. i have control of my bowel movements
and even my gas, meaning if i want to hold it in i can, but its just
always there waiting to come out. from the time i wake up to the time
i go to sleep. what can i do? ive tried otc products and nothing has
worked. i had an abdominal xray done in 2006 and it shows i have fecal
impaction but nothing else. what can i do? what can it be? its taking
over my life i am no longer happy or intimate with my fiance.

Dear Member,

You need an abdominal ultrasound and also upper and lower gastrointestinal
endoscopies to rule out a few conditions namely;

1.gallstones
2.colonic diseases
3.functional dyspepsia +/- Helicobacter pylori infection

if both tests turn out to be negative, then what you have could be Irritable
Bowel Syndrome.

In many parts of the world,this condition will need to be handled by a
Gastroenterologist.

while waiting to see one, you can take some otc like lactulose and
simethicone-based antacids

Hello Dr.Hamid, and thank you in advance.

      My problem will require a brief history session. By all accounts
I was a pretty healthy child. I rarely got sick, although I was
treated for athsma for a short period of time, I eventually no longer
needed athsma medication. It was rare that I had to go to the ER for
reasons other than stitches, broken bones, ect... I do have chronic
anxiety problems and am probably worrying too much about this. I was
never a very large kid as a child. My stools were generally hearty as
a child and adolescent.

  Once I hit puberty I put on weight very well, and at 19 I stand 5"8
and weigh in between 165 - 170 pounds with a fairly muscular build. I
began smoking at age 15 and around then developed a constant
stuffy/runny nose, which I've had ever since. However, in the four
years I've smoked I have rarely gotten sick apart from
colds/hangovers. I think a sore throat once or twice, maybe an
instance of bronchitis. My diet has always been awful, very high in
fats and fast foods. Also, since 17 I've been a heavy drinker and
marijuana smoker.

     As I approached age 18, especially after drinking I would have
runny stools and an increase in gas. Sometimes it's worse than others.
Anyway, just as I turned 19 about a month ago I had a pain in my lower
left abdomen after a night of excessive drinking. I stopped drinking
and smoking cigarettes afterwards, and after a few days the pain
disapeared. Shortly after I noticed a sensation under my right
shoulder blade. This would come and go, at its best a tingling
sensation, at its worse burning sensation. I began to have a
rediculous amount of gas, and pain after eating that would generally
stay true to my right side and flank. My stools lately have been
runny, sometimes with red strands and mucus. Othertimes, a more solid
stool. My appetite has decreased to the point where I eat once a day.
I went to see my doctor who ordered a blood test that came back normal
with a slightly elevated liver function. She said that it was likely
due to me suddenly quitting drinking and smoking, and my body was
having intense withdrawal fueled by anxiety. She refused to order an
x-ray or ultrasound or cystic fibrosis test, but instead perscribed
haldol for anxiety.

      The next day I went to the emergency room, complaining of the
same pain and was given another blood test that was normal, a chest
x-ray that was normal, a urinalysis that was normal, and a GI x-ray
that was normal. My symptoms match gallstones, which I've been told in
patients under 20 is a sign of an underlying condition like cystic
fibrosis. I match that up with my constantly runny nose, possibly
smoking related or not, my funky stools, past diagnosis of athsma and
loads of allergies, and I'm worrying that I could be one of those
diagnosed late. Do you suppose that with this constantly runny nose,
if I did have cystic fibrosis I would have more breathing problems
than I do? That I would have shown something abnormal on my chest
x-ray? Please, help me put my anxiety to rest and give me a possible
cause for my discomfort.

Dear Member,

As a Surgeon in Malaysia, I do not manage patients with Cystic Fibrosis and this
condition is very rare here.

It is possible that your symptoms might indicate problems related to Cystic
Fibrosis but the treatment is still the same, ie.symptomatic relief of your
ailments.Please visit your doctor frequently if the ailments keep coming despite
treatment.

Hello. My name is Jackie (27) and I just joined the group. I had a
couple questions and thought you may be able to answer.

I have had chest pains for years (mainly the past 3-5). It never lasts
long, so I never thought much of it. Recently, its been more often
(few times a month). And then yesterday, I had some leaking from a
nipple.

I am not pregnant, nor just had a baby (my youngest is 2 1/2 and I had
my tubes tied).

I was wondering what could cause it. I looked at WebMd but really
didn't get much help from it. Most of them, I ruled out.

Any information about what it could be, or what other symptoms I
should look at would be appreciated.

I didn't want to go to the doctor if it's nothing. Our insurance isn't
the best and I guessed it is probably something very minor.

1.There are many causes of chest pain.of course the most dangerous one is some
kind of heart disease.the only way to determine the cause of chest pain is to
get examined by a doctor who will then do some tests which include chest xray
and ecg

2.fluid coming out from the nipple is due to some condition in the breast which
can range from infection to cancer.you need to be examine by a Surgeon

i have been through several docs, on a urogolist, wjo will not do any thing, i am only desperate, cause the wont look for nerve damage or do any thing to fix it, painful sex etc, ive had some tests, but they say they can not find the route of the problem, been to a solictors to see if they can help, generally feel unwell and sleep all the time. tnks

night i wake out of my sleep with unbearable pain and swelling in my

now am experiencing the pain in the day time but for months it was only
at night only in one hand now it is in both hands and even during the

Dear Agi,

Burning and swelling hands usually means there is some inflammation going on.
A few inflammatory conditions which can occur in the hands are;

-rheumatoid arthritis
-osteoarthritis
-other rarer forms of arthritis
-infections of the hands/hand spaces

have you seen a doctor/orthopaedic surgeon?

while waiting to see a doctor you can take any of the NSAID painkillers

To ask the question I have to give you background first. My significant
other is, we assume, a pseudohermaphrodite. He is female by birth,
having female genitalia. However, he has high testosterone levels, no
menstrual cycle, builds muscle like a male, and gender reassigned
himself to a male. Based on my own research I discovered that some male
pseudohermaphrodites have testicles in their lower abdomen and I assume
he does because the area is sensitive to touch and he becomes aroused
by it.

My question is: If he does have testicles in his lower abdomen, and
because of his very, very high testosterone, is it possible that he
produces semen through his female genitalia? And if so, is there a
possibility that he is fertile and could get a woman pregnant?

If you can answer this question i'd really appreciate it.

Managing pseudohermaphrodite is a highly complex and specialised task.It
requires a combination of expertise namely ; Andrologist,geneticist,Plastic
Surgeon,Gynaecologist, Urologist etc.

Your question can only be answered after your partner is examined and tested by
all these experts.

However, just from the information above,if he has the testicles located in the
abdominal cavity,that situation is abnormal and potentially dangerous since the
testicles are highly susceptible to develop cancer.

i had a hysterectomy nearly three years ago, i have had bad pain, and
no sensation to wee and my bowel dont work proper, my left leg swells
up, they said it mite be nerve damage, but will not do any thing to

eyes are full of fluid and sweat because i want to wee, also have bad

You need to undergo many tests by a medical specialist preferably an Internal
Physician.
Those pain in the tummy could be related to previous surgery.You need to have
xrays,blood and urine tests for this.
Swollen leg could be due to problem in the tummy or even your heart or
kidney.You need all the above tests plus scan of your tummy.
Bagful of fluid around the eye usually occur in kidney failure.

Urgent attention is required here.

I'm a young woman (27) who has been suffering from knee stiffness and
pain for the last 7 years. I went to at least 10 different doctors,
all of whom say there is nothing wrong with me (they barely look at my
knee, just ask questions). I used to believe I was fine until this
month, when I started noticing small bruises with a red dot/some
reddening in the middle on my kneecap and on the side. This is
strange -- I no longer go jogging and I have not hit my knee. I am
worried and am wanting to get an MRI scan to make sure it is not
cancer. Can you please offer any advice? If you need to see a photo, I
have a (bad) photo of one of the bruises.

Was an Orthopaedic Surgeon among the 10 doctors you have seen?

I hope not!!!

You really need to consult an Orthopaedic Surgeon who will do a thorough
physical examination on your knee and perform various test which might include
xrays and MRI.
Meantime you can take any of the NSAID painkillers to buy time before the
consultation.

hello doc.
  i tore my acl about 3yrs ago and i delt with the pain for a while
but now its just way to intense the only way to describe it is
comparing it to a tooth ache where you either have to get a root
canal or pulled. now times that by 10. it wakes me up its just out
of control and i know i have to get surgery but i cant afford to be
out of work for 6-8months. do you have any suggestions on what to do

i am sending u this message from Iran.I have a problem in my ACL of left knee.2 years ago my ACL was injured in sport..but it was better very much after 5 months physioteraphy..but since 1 month ago i have had pain in my knee after a long walking just walking.. in my new MRI is written there is a partial tearing in my ACL.some Dr says  i should make a replacement surgery but some Dr says my ACL doesnt tear perfectly and it can be better with physioteraphy..

I am wondering if juvenile arthritis and tendinitis are treatable.

1.Juvenile Arthritis is a variant of Rheumatoid Arthritis.The exact cause is
unknown. Definitely it is treatable to control the symptoms BUT it is not
curable.ie.the disease will be with the patient forever.

2.Tendinitis is a condition whereby there is inflammation of the tendon and

she went out side and fell down 4 stairs ( wood). She couldn't move
and still is having hard time breathing. We raised her arms over her
head and was able to breath, but it hurt. Were worried if she did some
damages her lungs, is that likely?

Dear Member,

I suggest you bring her to the emergency department to have the xrays of the
chest and backbones done to rule out any damage to the lungs or other structures
in the thorax or any bones fracture.

Hi I was wondering if you could help me. For the last couple of weeks I have had an almost constant headache. Sometimes just above one eye, sometimes an all over headache. I have also been seeing things in my right eye. Kind of a bright light down the right hand side of my right eye that moves downwards and then dissapears. Maybe once or twice a day this happens. And last night when I shut my eyes I got bright flashing lights in both eyes and a headache as well. I was wondering if you had any idea what this could be? I have a chalazion in my left eye and in the past have suffered from migraines (but not for a while) I got my eyes checked last about a year ago.

Hi,

I dont know what to do and im very scared. I am a 22 year old female im
5 foot 7 and i weigh 103 pounds. I was never like this. I just keep
losing despite how much or what i eat. My breast bones are actually
showing and it really freaks me out. I dont do this to me purposly. I
have been under enourmous stress every single day for the last 2
months.Stress which i find unbearable at times. I dont know if that
plays a role. I love junk food and i eat. Whats wrong with me...I
losing weight RAPIDLY and im very scared...

Dear Grace,

Losing weight without having intention despite eating normally raise the
possibility of 2 conditions;

1. hyperthyroidism - hyperactive thyroid gland activity

2. cancer

you need to undergo thorough specialist investigations to rule out both
conditions.

Hi Doctor,


     Iam roshan,male,25aged working for an IT company. My problem is my
voice is not so hard i mean , its not a male voice , always when i
call strangers thay used to wish me like (yes madam) . so let me plz
know how come i can over come this problem. Is there any madicine or
any exercise so that i can improve my voice little more harder ..

Dear Roshan,

the quality of our voice is usually affected by the structures in our throat.I
advise to consult an Ear Nose and Throat (ENT) Surgeon for a check up
initially.If no structural abnormality is found, then perhaps you should see a
Speech Pathologist(Therapist) to help you adjust the pitch of your voice.

Dear Dr. Hamid.
A 66 years old chinese gentleman was admitted with complaint of per
rectal bleeding associated with altered bowel habits and weight loss
for the past 2 weeks. He also complaint of having dysuria, hemturia and
passing fecal matter trough his genitalia. Upon further investigation,
his hemoglobin levels was 8.4 g/dl with CEA level of 9.1. Renal profile
showed UREA levels of 23. Colonoscopy was done and reported that there
was a mass just above the dentate line and it obstruct penetration of
colonoscope hence assesment of deeper area of rectum and colon
unsucessful. Upon insertion of continuous bladder drainage, there were
fecal matter passing trough his genitalia.
1) What was the cause of recto-vesicle fistula in elderly?
2) How would you manage this gentleman?

Dear Member,

1.In this situation, the rectovesical fistula(abnormal connection between the
rectum and the bladder)is most probably due to the rectal cancer which has
infiltrated through the wall of the rectum and then through the adjacent wall of
the bladder.(The wall of the rectum and the wall of the bladder is in close
proximity anatomically speaking).

2.The Management;

i.diagnosis of the cancer by getting the biopsy of the tumour at
colonoscopy.confirmation of cancer from the histological examination

ii. staging of the rectal cancer by ct scan of the pelvis and abdomen.local
pelvic infiltration, secondaries in the pelvic lymph nodes and paraortic lymph
nodes and in the liver will be detected to determine the stage.
advanced cancer will be managed conservatively focussing to alleviate
symptoms(palliative).
early or localised cancer will entail aggressive surgical management to cure the
patient from the cancer.
in view of the fistula, it is assumed that the patient already has advanced
rectal cancer and hence conservative management is the philosophy of management
here.
the ct scan will also provide the urogram information about obstructive
uropathy(blockage of renal system)in view of high urea

iii.since there is connection between the bladder and the rectum,and feces goes
into the urine hence infecting it,antibiotics need to be given over prolong
period

iv.since the patient is anaemic, blood tranfusion is required.

v.if the ureter(s) is/are blocked, internal ureteric stent(s) should be inserted
to improve the renal function

vi.the tumour should be managed by using radiotherapy+chemotherapy

Glucosamine sebagai supplement hanya perlu diambil oleh orang yang sudah separuh
umur 40an ke atas untuk mengelakkan penyakit sendi(osteoarthritis).
Glucosamine yang semulajadi masih banyak bagi orang yang lebih muda.
