Online Survey for Adults who are Deaf or Hard-of-Hearing on Experiences
with Health Care Providers
We are seeking adults who are deaf/Deaf or hard-of-hearing and/or who
have voice or speech impairment to participate in a brief online survey
to provide input to the development of educational materials for medical
professionals.
With a grant from the National Institute on Deafness and Other
Communication Disorders of the federal government's National Institutes
of Health (NIH), Sociometrics ( <outbind://120/www.socio.com>
www.socio.com) in Los Altos, CA is developing a set of computer-based
educational materials for health care providers. These materials will
help increase health care providers' ability to communicate effectively
with-and provide appropriate care for-patients who are deaf/Deaf or
hard-of-hearing and/or who have voice, speech, or language impairment,
and their families.
ABOUT THE SURVEY: The survey will take about 5-10 minutes to complete
and will ask questions about your experiences with healthcare providers.
The survey is anonymous-- you will not need to provide your name or
contact information to participate.
"THANK YOU" PRIZE DRAWING: After you take the survey, you will be given
the option to enter a drawing to win one of three $75 cash prizes. The
drawing is being offered to thank survey participants for contributing
to the project. The contact information collected for the drawing will
NOT be linked to the surveys and will be used ONLY
for the drawing, and not for any other purpose.
