Hi,
Just a word of advice... be specific about what you want. I had not
been to the doctor in 24 years. I needed a health form filled out. I
was so scared. I didn't want to go. So I asked to have the form
filled out. However, the doctor found out that I had not been in
such a long time. So I ended up getting the complete exam. I was
such a nervous wreck.

If you want the least invasive exam, let them know that. Let them
know about your fears. Let them know that maybe later you will get
an exam but right now you're not ready.
